Job Title: Social Services Assistant
Facility: Melbourne Healthcare and Rehabilitation
Location: Melbourne, Florida
Shift: Full-Time | 8:00 AM – 4:30 PM
Salary: $20–$23 per hour (based on experience)

We are currently seeking a dedicated Social Services Assistant to support the psychosocial well-being of our residents and contribute to a positive facility environment. Key Responsibilities:

• Assist residents with psychosocial needs, including adjustment to facility life, emotional support, and discharge planning.

• Conduct assessments and collaborate on care planning for new and existing residents as part of the interdisciplinary team.

• Maintain accurate and timely documentation in compliance with state and federal guidelines.

• Support and advocate for residents' rights, mediating concerns or grievances as needed.

• Coordinate social services interventions and refer residents to appropriate external agencies (e.g., mental health, hospice, housing).

• Assist with discharge planning, including coordinating home services, equipment, and communication with families or caregivers.

• Participate in care conferences and collaborate with nursing, therapy, dietary, and medical staff.

• Help plan and implement social programs and activities to support residents’ mental and emotional well-being.

• Maintain confidentiality and promote resident dignity and respect in all interactions.

Qualifications:

• High school diploma or equivalent required; associate or bachelor's degree in social work, psychology, human services, or a related field preferred.

• Previous experience in a healthcare setting, especially long-term care or skilled nursing, preferred.

• Knowledge of state and federal resident care regulations (e.g., OBRA, HIPAA).

• Strong communication, organizational, and documentation skills.

• Empathetic, patient, and supportive attitude.
