As a Charge Nurse 6A-2P at Ivy Creek Wellness & Rehabilitation, you will have the opportunity to provide outstanding care to our patients and ensure that patient needs are met in accordance with standards of practice, physician orders, and center policies.

About the Role:

As a Charge Nurse on our 6A-2P team, you will be responsible for:

• Providing direct patient care and assisting with daily living activities

• Monitoring patient conditions and reporting any changes to physicians and other healthcare professionals

• Documenting patient information and updating medical records

• Collaborating with other healthcare team members to ensure seamless care coordination

Qualifications:

• Current LVN/LPN license in practicing state

• Current BLS/CPR certification or willingness to acquire one

• Experience in nursing management is preferred but not required

Responsibilities:

• Follows established procedures for charting and incident reporting

• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction

• Documents any change in patient conditions

• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs

• Documents and notifies applicable departments of patient room changes, LOA, new admits, or discharges
