Fairport Rehab & Nursing Center, a skilled nursing and rehabilitation facility in Fairport, New York, is seeking a compassionate, organized, and detail-oriented Admissions Nurse (RN/LPN) to join our team. We are looking for a nurse with strong clinical judgment, excellent communication skills, and a commitment to helping residents and their families experience a smooth and welcoming transition into our facility. RN licensure is preferred; qualified LPN candidates will also be considered.

The Admissions Nurse plays an important role in coordinating new resident admissions, reviewing clinical information, communicating with referral sources, and collaborating with the interdisciplinary team to ensure residents' care needs are identified and addressed from the start.

Key Responsibilities

• Coordinate and oversee the admissions process from initial referral through resident arrival and post-admission follow-up.

• Review referral information, medical records, physician orders, medications, and treatment plans for accuracy and completeness.

• Conduct admission nursing assessments and help identify residents' clinical needs and appropriate care requirements.

• Communicate with physicians, hospitals, referral sources, residents, families, and other healthcare professionals to facilitate timely admissions.

• Collaborate with the Director of Nursing (DON), Assistant Director of Nursing (ADON), nursing staff, and interdisciplinary team to support continuity of care.

• Complete admission paperwork, clinical documentation, and electronic medical record entries in PointClickCare.

• Ensure admission records are accurate, organized, and completed in accordance with facility policies and applicable regulatory requirements.

• Provide guidance and support to residents and families, answering questions and helping ease the transition into the facility.

• Participate in an on-call rotation to support admission coordination and facility needs outside regular business hours, as required.

• Perform additional duties as assigned by the DON or ADON.

Qualifications

• Current, active New York State Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in good standing.

• RN preferred; LPN candidates with relevant experience are encouraged to apply.

• Previous experience in skilled nursing, long-term care, rehabilitation, hospital admissions, case management, or a related healthcare setting preferred.

• Experience with PointClickCare or another electronic medical record (EMR) system is a plus.

• Strong clinical assessment, organizational, communication, and interpersonal skills.

• Excellent attention to detail and the ability to manage multiple priorities in a fast-paced environment.

• Ability to work independently while collaborating effectively with residents, families, referral sources, and the interdisciplinary team.

• Flexibility to participate in an on-call rotation based on facility needs.

Compensation & Schedule

• RN Salary: $90,000 per year.

• Schedule: Monday–Friday, day shift.

• On-Call: Participation in a rotating on-call schedule is required.

• Hours may vary depending on admissions activity and facility needs..
