As a Wound Care Nurse, you will play a vital role in the delivery of high-quality care to our residents, providing wound management and treatment to ensure optimal healing and prevent re-hospitalization. Responsibilities:

• Provide wound care management and treatment to residents, including debridement, dressing changes, and wound assessment

• Conduct comprehensive wound assessments to identify areas for improvement and develop individualized treatment plans

• Collaborate with other healthcare professionals to ensure seamless care coordination and effective communication

• Monitor and document resident progress, updating treatment plans as necessary to ensure optimal healing and prevent complications

• Educate residents, families, and healthcare teams on wound care best practices and prevention strategies

• Participate in multidisciplinary team meetings to discuss resident care and develop comprehensive plans to achieve desired outcomes

Requirements:

• Current licensure as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) in the State of Illinois

• Minimum 1 years of experience as a Wound Care Nurse

• Strong knowledge of wound care principles, practices, and best practices

• Excellent communication and interpersonal skills

• Ability to work effectively in a fast-paced environment
