Cedar Haven is seeking a compassionate and motivated Physical Therapist (PT) to join our therapy team. This is an excellent opportunity for a PT who is passionate about providing quality care and helping residents achieve their highest level of function, mobility, and independence.

Position Overview

The Physical Therapist will evaluate residents, develop individualized treatment plans, and provide skilled physical therapy services in a skilled nursing and rehabilitation setting. The PT will work closely with residents, families, nursing staff, and other members of the interdisciplinary team to achieve positive clinical outcomes and promote quality of life.

Key Responsibilities

• Evaluate residents and develop individualized physical therapy treatment plans

• Provide skilled physical therapy services based on resident needs and established goals

• Implement therapeutic exercises, mobility training, balance activities, transfers, and functional training

• Monitor resident progress and modify treatment plans as appropriate

• Maintain accurate and timely clinical documentation

• Communicate effectively with residents, families, physicians, nursing staff, and other members of the care team

• Participate in interdisciplinary care planning and team meetings as appropriate

• Promote resident safety, mobility, independence, and quality of life

• Maintain compliance with applicable state and federal regulations and facility policies

• Provide compassionate, resident-centered care while maintaining a positive and professional environment

Qualifications

• Current Physical Therapist license in the state of Pennsylvania

• Graduate of an accredited Physical Therapy program

• Current CPR certification or ability to obtain

• Previous experience in skilled nursing, rehabilitation, or long-term care preferred

• Strong clinical, communication, and interpersonal skills

• Ability to work effectively as part of an interdisciplinary team
