Grandview Nursing & Rehabilitation Center is seeking a compassionate, dedicated, and motivated Part-Time Physical Therapist (PT) to join our rehabilitation team!

Responsibilities

• Evaluate residents and develop individualized physical therapy treatment plans based on their needs and goals.

• Provide skilled physical therapy services to improve mobility, strength, balance, and overall functional independence.

• Assist residents in achieving their highest possible level of independence and quality of life.

• Collaborate with nursing staff, physicians, occupational therapists, speech-language pathologists, and other healthcare professionals to ensure coordinated resident care.

• Maintain accurate and timely clinical documentation using Net Health.

• Participate in interdisciplinary care planning and resident progress discussions.

• Educate residents, families, and caregivers on therapeutic exercises, mobility techniques, and safety strategies.

• Ensure compliance with all applicable state, federal, and facility regulations.

Qualifications

• Graduate of an accredited Physical Therapy program.

• Current Pennsylvania Physical Therapist (PT) license or eligibility for licensure.

• Previous experience in a SNF/LTC setting is preferred but not required.

• New graduates are encouraged to apply!

• Strong communication, organizational, and interpersonal skills.

• Compassionate, patient-centered approach to care.

• Ability to work independently while contributing to a collaborative rehabilitation team.

• Familiarity with Net Health and PCC is a plus, but not required.
