About the Role:

We are seeking a highly motivated and compassionate LPN/RN to join our PRN team. As an LPN/RN with Merriam Gardens Healthcare & Rehabilitation, you will be responsible for providing exceptional patient care, working collaboratively with our interdisciplinary care team, and contributing to our mission to provide the best care possible. This is a per-diem opportunity, which means you will work flexible hours and have the autonomy to choose when you want to work.

Responsibilities:

• Provide direct patient care, including administration of medications, wound care, and vital sign monitoring

• Develop and implement individualized care plans in collaboration with our interdisciplinary care team

• Monitor patient progress, identifying and reporting changes or concerns to the care team

• Collaborate with other healthcare professionals to ensure comprehensive care

• Maintain accurate and detailed records of patient care and treatment

• Participate in quality improvement initiatives to enhance patient outcomes

• Demonstrate a commitment to patient-centered care and a passion for delivering exceptional service
