Job Summary:

Carease Health - OH is seeking a compassionate and dedicated Mental Health Provider to join our Collaborative Care team as a Care Companion/Social Worker. In this part-time role, you will provide emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities. Responsibilities:

• Visit designated facilities in accordance with the schedule set with your supervisor to assess residents' emotional, cognitive, and psychosocial needs.
• Conduct assessments to identify the needs of residents and implement short-term, solution-focused therapy and supportive interventions to address immediate concerns.
• Document visit notes and maintain detailed records of interactions and interventions after each visit.
• Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP.

Qualifications:

• Associate Degree in Social Work, Psychology, Counseling, or another mental health–related field.
• Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings.
• Strong communication, documentation, and relationship-building skills.

Salary: $17,000 - $19,000 ANNUALLY
