Responsibilities:

• Conduct social support assessments to identify patients' emotional, social, and vocational needs

• Develop and implement individualized plans to address patients' needs and goals

• Collaborate with interdisciplinary teams to ensure comprehensive care and support

• Provide counseling and education to patients and their families on issues related to aging, disability, and chronic illness

• Monitor patients' progress and adjust plans as needed to ensure optimal outcomes

• Maintain accurate and thorough records of patient assessments, plans, and progress

Requirements:

• Bachelor's degree in Social Work, Psychology, or related field (n/a)

• 0 years of experience (minimum)

• Strong communication, interpersonal, and problem-solving skills

• Ability to work independently and as part of a team

• Compassion, empathy, and understanding of the needs of vulnerable populations

• Familiarity with social support services and community resources
