Registered Dietician

We are seeking a dedicated and compassionate Registered Dietitian to join our interdisciplinary care team in a full-time capacity. This position is split between Bradford Manor Nursing and Rehab in Bradford, PA, and Kinzua Nursing and Rehab in Warren, PA. The ideal candidate will be passionate about improving the health and quality of life for residents in a skilled nursing facility (SNF) setting.

Key Responsibilities:

• Provide nutritional assessments, care planning, and ongoing evaluation of residents' nutritional needs.

• Develop and implement individualized nutrition plans in coordination with nursing and medical staff.

• Monitor residents’ clinical status and adjust nutrition plans as needed.

• Participate in care plan meetings and collaborate with the interdisciplinary team.

• Ensure compliance with federal, state, and local regulations, as well as facility policies.

• Educate staff, residents, and families on nutrition-related topics.

• Conduct regular audits of dietary services and assist with menu planning and review.

• Travel between the Bradford and Kinzua facilities as scheduled.

Qualifications:

• Bachelor’s degree in Nutrition, Dietetics, or a related field 

• Registered Dietitian Nutritionist (RDN) credential required.

• Licensure in the state of Pennsylvania

• Minimum of 1 year of clinical experience, preferably in a SNF or long-term care setting.

• Strong communication, organizational, and clinical documentation skills.

• Reliable transportation for travel between locations.
