Responsibilities:

• Supervise and support nursing staff during shifts, ensuring adherence to clinical protocols and safety standards

• Coordinate patient care activities, including assessments, wound care, medication administration, IV infusions, and documentation using EMR systems like MatrixCare

• Oversee patient assessments, vital signs, and physical examinations to develop individualized care plans

• Ensure compliance with HIPAA regulations and maintain accurate medical records and documentation for all patient interactions

Requirements:

• Current LPN licensure in the state of Tennessee; additional certifications such as Basic Life Support (BLS) or Cardiopulmonary Resusitation (CPR) required.

• Strong knowledge of anatomy, physiology, medical terminology, and diagnostic evaluation techniques.

• Experience managing cases involving hospice & palliative medicine or behavioral health is highly desirable.

• Compassionate approach to working with individuals across age groups including geriatrics, or patients with developmental disabilities or dementia care needs

• Ability to work in a fast-paced environment with frequent interruptions

• Strong communication and leadership skills
