Care Coordinator

Partners In Care is growing, and we’re looking for a compassionate, dependable, and motivated Care Coordinator to join our Chico team.

This isn’t a position where you’ll spend every day behind a desk. You’ll work directly with seniors and people with disabilities, their families, and our caregivers throughout Chico, Oroville, and Yuba City, helping coordinate care and making sure our clients receive the support they need.

As a Care Coordinator, you’ll play an important role in building relationships with our clients and caregivers while helping ensure consistent, quality care throughout our service areas.

Your Day May Include:

• Checking in with clients in their homes and building positive, supportive relationships

• Completing client assessments and follow-up visits

• Introducing caregivers to clients and providing on-site training and guidance

• Supporting and communicating with caregivers

• Working closely with clients, their families, and company staff to coordinate care

• Visiting clients who have been transferred to hospitals, skilled nursing facilities, or other care settings

• Assisting with caregiver recruiting, staffing, and company orientation

• Representing Partners In Care at community events, resource fairs, vendor fairs, sponsored events, and other outreach opportunities

You’ll Be a Great Fit If You:

• Have experience providing or coordinating in-home care for seniors and/or people with disabilities

• Have Care Coordinator or management experience, preferably at least one year

• Are organized and able to manage multiple priorities effectively

• Have excellent communication and customer service skills

• Can complete work accurately and efficiently in a fast-paced environment

• Work well independently while also being a strong team player

• Are comfortable using Microsoft Word, Excel, and Outlook

• Are able to travel throughout Chico, Oroville, and Yuba City

Position Details:

• Job Type: Full-time

• Pay: $19.00–$25.00 per hour

• Location: Chico, CA

• Work Location: In person

• Travel: Daily travel throughout Chico, Oroville, Yuba City, and surrounding service areas

• Company Vehicle: Provided for work-related day travel

• Experience: 1 year of Care Coordinator experience preferred

• Ability to Commute: Chico, CA required

• Ability to Relocate: Must relocate to Chico, CA before starting work if applicable
