Ascend Hospice is seeking a compassionate and experienced Chaplain to provide spiritual and emotional support to hospice patients and their families. Join an interdisciplinary team dedicated to honoring each patient’s beliefs, values, and wishes while providing comfort through end-of-life care.

Location: Somerville, NJ 08876
Coverage Area: Primarily Somerset, Warren, Hunterdon, and Union counties, as well as parts of Middlesex County. Travel throughout the assigned service area is required.

Key Responsibilities:
• Provide individualized spiritual counseling that respects each patient’s and family’s beliefs, traditions, and preferences.
• Complete timely spiritual assessments and develop individualized spiritual care plans.
• Support patients and families of all backgrounds, including those with no religious affiliation.
• Collaborate with the interdisciplinary team to coordinate care and address spiritual and emotional needs.
• Provide bereavement support and follow-up services to families.
• Respond to patient and family needs with empathy, professionalism, and respect.

Qualifications:
• Master’s degree in Divinity.
• 2–4 years of chaplain experience, preferably in hospice or end-of-life care.
• Completion of at least one unit of Clinical Pastoral Education (CPE).
• Ability to provide inclusive, non-denominational spiritual care.
• Strong communication, interpersonal, and collaboration skills.
