Schedule: Per Diem / On Call | Weekdays, 4:00 PM–12:00 AM, and weekends as needed
Pay Rate: $55.00–$60.00 per hour

Spring Creek Rehabilitation & Nursing Care Center is seeking an experienced and compassionate Registered Nurse (RN) Supervisor to provide clinical leadership and oversee nursing operations on a per diem, on-call basis. Coverage includes weekday evening shifts from 4:00 PM to midnight and weekends as needed.

This opportunity is ideal for an RN who enjoys leading a team, supporting quality resident care, and working in a fast-paced skilled nursing environment.

Key Responsibilities

• Oversee and coordinate resident care during assigned shifts.

• Supervise nursing staff and provide clinical guidance and support.

• Monitor resident conditions and respond promptly to changes in health status.

• Collaborate with physicians and interdisciplinary team members to support individualized care plans.

• Ensure compliance with facility policies, procedures, and applicable regulatory standards.

• Promote resident safety, quality care, and respectful communication with residents and families.

• Provide hands-on nursing care and clinical support as needed.

• Address nursing concerns and assist staff with problem-solving.

• Ensure accurate documentation and effective communication between shifts.

Qualifications

• Active New York State Registered Nurse (RN) license.

• Minimum of two years of nursing experience.

• Previous supervisory or charge nurse experience preferred.

• Skilled nursing or long-term care experience preferred.

• Strong clinical judgment, leadership, communication, and organizational skills.
