We are seeking a compassionate and skilled Physical Therapist to join our multidisciplinary team. As a Physical Therapist at Polaris Rehabilitation and Care Center, you will have the opportunity to make a meaningful impact on the lives of our patients, providing individualized physical therapy services to achieve optimal physical function and well-being.

Responsibilities:

• Develop and implement personalized physical therapy treatment plans for patients to achieve specific goals and outcomes

• Conduct thorough evaluations and assessments to identify physical therapy needs and develop treatment plans

• Deliver high-quality physical therapy services, including but not limited to exercises, modalities, and manual therapy

• Communicate effectively with patients, families, and healthcare team members to ensure seamless care transitions and coordination

• Document patient progress and treatment plans accurately and in a timely manner

• Participate in quality improvement initiatives to enhance patient outcomes and facility operations

Requirements:

• Physical Therapist degree from an accredited institution

• A strong focus on patient-centered care

• State licensure in good standing
