We’re hiring an Admissions Coordinator to manage referrals, tours, and the full admission process from first contact to move-in day.

What you’ll do

• Manage incoming referrals from hospitals, physicians, and community partners

• Review clinical info and verify insurance and payer eligibility

• Give tours and walk families through the admission process

• Coordinate admissions with nursing, therapy, social services, and business office

• Complete admission paperwork accurately and on time

• Build and maintain relationships with hospital case managers and referral sources

• Track census and referral activity and report to leadership

What you’ll need

• Admissions, case management specifically in a SNF setting.

• Working knowledge of Medicare, Medicaid, and managed care

• Strong communication and customer service skills

• Organized, detail oriented, and comfortable with a fast pace

• Compassionate approach with residents and families
