Position Overview

We are seeking a well-connected, relationship-driven Nursing Home Liaison to develop and maintain relationships with nursing homes, rehabilitation centers, hospitals, discharge planners, social workers, and other healthcare professionals within the local market.

Candidates must have an established network of contacts and strong relationships within the local healthcare community. The ideal candidate will be able to leverage these existing connections to build partnerships, generate referrals, and expand our presence in the market.

Key Responsibilities

• Develop and maintain relationships with nursing homes, skilled nursing facilities, rehabilitation centers, hospitals, and other healthcare providers.

• Leverage existing local connections to establish partnerships and generate new referral opportunities.

• Serve as the primary point of contact between the organization and healthcare facilities.

• Conduct regular outreach and in-person visits to local facilities and key decision-makers.

• Identify opportunities to strengthen referral relationships and expand the organization's presence in the local market.

• Collaborate with internal teams to ensure effective communication, coordination, and follow-up with referral sources.

• Monitor referral activity and maintain strong, ongoing relationships with community partners.

Required Qualifications

• Established connections within the local healthcare market are required.

• Existing relationships with nursing homes, skilled nursing facilities, hospitals, rehabilitation centers, social workers, discharge planners, or healthcare administrators.

• Previous experience in healthcare liaison services, referral development, business development, or healthcare marketing.

• Strong networking, communication, and relationship-building skills.

• Knowledge of the local healthcare landscape and key referral sources.

• Ability to independently develop opportunities and maintain productive relationships with healthcare partners.

• Willingness to travel locally for facility visits and relationship management.

We are looking for someone who can leverage existing connections to build trust, develop referral partnerships, and generate results from the outset, rather than someone who needs to build a network from scratch.
