Position Overview

We are seeking a detail-oriented and organized Home Health Intake & Eligibility Coordinator to review incoming patient referrals, verify insurance eligibility, and determine whether the agency can appropriately service each referral.

Home Health Intake & Eligibility Coordinator Key Responsibilities

• Review incoming referrals and supporting clinical documentation for completeness and accuracy.

• Verify insurance coverage, benefits, eligibility, and authorization requirements.

• Determine whether the agency can service each referral based on payer requirements, patient needs, service area, and staffing availability.

• Identify missing documentation or potential barriers to accepting referrals and follow up as needed.

• Communicate with referral sources, insurance providers, and internal clinical and administrative teams.

• Track referral status and ensure timely follow-up on pending cases.

• Maintain accurate referral records and ensure patient information remains confidential.

Home Health Intake & Eligibility Coordinator Qualifications

• Previous experience in healthcare intake, referral coordination, insurance verification, or a related role preferred.

• Familiarity with insurance eligibility and home health referral processes.

• Strong organizational skills and attention to detail.

• Excellent communication and follow-up skills.

• Ability to manage multiple referrals in a fast-paced environment.
