About the Role

As an experienced home health professional, you will play a vital role in supporting the health and well-being of our clients. Your responsibilities will include:

• Conducting comprehensive assessments of clients' needs and developing personalized care plans

• Providing high-quality nursing care, including medication management, wound care, and more

• Collaborating with our interdisciplinary team, including therapists, social workers, and caregivers, to ensure seamless care coordination

• Educating clients and their families on disease management, medication adherence, and self-care techniques

• Maintaining accurate and detailed records of client care, including progress notes and treatment plans

Requirements

• 1+ year of experience in home health care or a related field

• Strong communication and interpersonal skills

• Ability to work independently and as part of a team

• Flexibility to work a FULL_TIME schedule, including weekends and holidays
