A beautiful skilled nursing facility located in Brentwood, NY is seeking a strong Director of Social Work with a background in long-term care. This role is responsible for supporting residents and families, coordinating care planning, and ensuring compliance with regulatory requirements.

Responsibilities of the Director of Social Work-Nursing Home:

• Plans and directs a comprehensive social services program which provides support services for residents and families.

• Responsible for coordinating all admissions and discharges to ensure facility census is kept at its maximum capacity.

• Participates in an interdisciplinary team approach to better serve the needs of the facility population.

Qualifications:

• MUST have prior experience in long-term care as a Director of Social Work

• Strong communication and documentation skills

• Knowledge of discharge planning and care coordination

• Familiarity with state and federal long-term care regulations
