About the Role:
As an MDS Coordinator, you will be responsible for the completion and coordination of the Minimum Data Set (MDS) process, ensuring accurate and timely submission of assessments, changes, and revisions to the nursing facility's quality measures. Your expertise will help maintain compliance with federal and state regulations, while fostering a positive and efficient working environment. With a focus on patient-centered care, you will work collaboratively with our interdisciplinary team to provide exceptional care to our residents.

Responsibilities:

• Complete and coordinate the MDS process, ensuring accurate and timely submission of assessments, changes, and revisions to the nursing facility's quality measures

• Develop and maintain relationships with the interdisciplinary team to ensure effective communication and collaboration

• Participate in MDS training and knowledge-sharing initiatives to stay up-to-date on best practices and regulatory changes

• Monitor and report MDS data to regulatory agencies and internal stakeholders

• Collaborate with the Quality Assurance Coordinator to identify and implement process improvements

• Participate in staff education and training programs to enhance skills and knowledge

Requirements:
