Licensed Practical Nurse (LPN) – Palliative Care

NJPalCare Solutions
Location: New Jersey – Skilled Nursing & Long-Term Care Facilities
Salary Range: $60,000 – $80,000 annually, based on experience and qualifications

About the Role

NJPalCare Solutions is seeking a compassionate, highly organized Licensed Practical Nurse (LPN) to join our growing palliative care team serving patients in skilled nursing and long-term care facilities.

The LPN serves as the on-site clinical partner to our Advanced Practice Providers (APPs) and plays a critical role in extending palliative care to more patients efficiently and consistently. Working directly at the bedside, the LPN gathers key clinical information, engages patients and families, coordinates with facility staff, and facilitates visits with an APP, either in person or through telehealth.

This is an ideal opportunity for an LPN who enjoys patient interaction, works well independently, is comfortable with technology, and wants to be part of a growing team focused on improving care for medically complex patients.

What You'll Do

Patient Care & Clinical Support

  • Conduct structured bedside visits with patients identified for palliative care evaluation and follow-up.

  • Gather and document relevant clinical information, including symptoms, changes in condition, functional status, medications, and other information needed by the APP.

  • Identify patient and family concerns and communicate findings to the APP.

  • Support advance care planning and goals-of-care discussions within the LPN scope of practice and established clinical protocols.

  • Recognize changes in patient conditions and escalate concerns appropriately.

  • Support ongoing palliative care patients according to established care plans and follow-up schedules.

APP & Telehealth Partnership

  • Serve as the APP's eyes and ears at the bedside, particularly when the APP is working remotely.

  • Prepare patients and gather relevant clinical information before APP encounters.

  • Facilitate telehealth visits using approved technology and remain with patients as appropriate during encounters.

  • Assist APPs by providing bedside observations and clinical updates.

  • Coordinate follow-up care and recommendations with facility staff.

Facility Collaboration

  • Develop strong working relationships with nursing leadership, floor nurses, social workers, physicians, and other facility staff.

  • Participate in facility huddles and clinical discussions to identify patients who may benefit from palliative care.

  • Help ensure patients receive timely evaluations and appropriate follow-up.

  • Serve as a professional and dependable on-site representative of NJPalCare Solutions.

Documentation & Care Coordination

  • Complete accurate and timely clinical documentation.

  • Maintain organized patient lists, completed visits, follow-up needs, and outstanding clinical items.

  • Follow established workflows for patient identification, escalation, APP
