We are seeking an experienced Nurse Practitioner (NP) to join our team at Carease Health in Paris, TN. As a leading nationwide preventative healthcare company, we specialize in reducing hospitalizations through Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM).

About the Role:

As a Nurse Practitioner, you will be responsible for delivering high-quality preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation. You will build strong relationships with the facility, maintain a patient-centered approach, and collaborate with our team to provide exceptional care.

Responsibilities:

• Provide on-site visits and remote care to residents in skilled nursing facilities

• Conduct patient assessments, develop treatment plans, and collaborate with healthcare teams

• Utilize RPM, CCM, and CoCM to monitor patients' vital signs, detect abnormalities, and address mental health needs

• Maintain accurate and timely documentation, supported by a dedicated scribe team

• Build strong relationships with facilities, patients, and families

Requirements:

• Active NP or PA license (in the state of practice)

• 1+ year of experience in Geriatric care and documentation review

• Excellent communication, collaboration, and teamwork skills
