Nurse Risk Manager and Infection Preventionist

White Oaks Rehabilitation & Nursing Center is seeking a highly motivated, detail-oriented, and experienced Risk Manager / Certified Infection Preventionist to join our leadership team.

The ideal candidate will be a licensed RN or LPN with strong clinical knowledge, excellent organizational skills, and a passion for resident safety, infection prevention, regulatory compliance, and continuous quality improvement.

This position plays a critical role in promoting a safe environment for residents, staff, and visitors while ensuring compliance with CMS and New York State Department of Health (NYSDOH) requirements.

Key Responsibilities

Risk Management & Resident Safety

• Oversee the facility's Risk Management Program to identify, investigate, and minimize potential risks.

• Review, investigate, and track resident accidents, incidents, injuries, falls, and other safety concerns.

• Ensure timely reporting, documentation, investigation, and follow-up of incidents in accordance with facility policies and regulatory requirements.

• Identify trends and implement corrective actions to prevent recurring incidents.

• Collaborate with nursing leadership and department heads to improve resident safety.

• Participate in Quality Assurance and Performance Improvement (QAPI) initiatives.

• Assist with regulatory surveys, investigations, Plans of Correction, and ongoing compliance monitoring.

Infection Prevention & Control

• Oversee and maintain the facility's Infection Prevention and Control Program.

• Conduct infection surveillance, tracking, trending, and reporting.

• Monitor compliance with standard and transmission-based precautions, hand hygiene, and appropriate PPE use.

• Investigate potential outbreaks and coordinate prevention and containment measures.

• Provide infection prevention education, staff training, and competency assessments.

• Monitor antibiotic stewardship initiatives in collaboration with clinical leadership.

• Ensure compliance with applicable CMS, CDC, OSHA, and NYSDOH infection prevention requirements.

• Conduct infection control rounds, audits, and follow-up evaluations.

• Serve as a resource to staff regarding infection prevention best practices.

Qualifications

• Current, unrestricted New York State RN or LPN license.

• Infection Preventionist Certification required.

• Previous experience in a skilled nursing facility or long-term care setting preferred.

• Experience in infection prevention, risk management, quality assurance, or regulatory compliance strongly preferred.

• Strong understanding of CMS and NYSDOH regulations.

• Excellent communication, documentation, organizational, and problem-solving skills.

• Ability to work independently and collaboratively with an interdisciplinary leadership team.

• Strong computer skills and experience with electronic medical records preferred.
