We are seeking a dedicated and compassionate Social Worker to join our team at Fort Lauderdale Health & Rehabilitation Center, a Skilled Nursing Facility that is dedicated to Setting standards in Rehabilitation and Skilled Nursing Care. As a Social Worker, you will play a vital role in ensuring the social and emotional well-being of our residents.

About the Role

As a Social Worker, you will assist in planning, organizing, implementing, evaluating, and directing the overall operation of our Social Services Department in accordance with current federal, state, and local guidelines and regulations. Your responsibilities will include:

• Participate in discharge planning, development and implementation of social care plans and resident assessments.
• Interview residents and families to obtain social history.
• Work with community resources to ensure the patient has a safe plan when they transition out of the facility.
• Possess the ability to seek out new methods and principles and be willing to incorporate them into existing social services.

Requirements

• Should have a minimum, one (1) year, social work experience in a health care setting working directly with individuals.
• MEDICAID experience in an LTC/SNF setting preferred.
