At Chestnut Park Rehabilitation & Nursing Center, we strive to deliver exceptional care and services to our residents, and we're seeking a dedicated and compassionate SLP Speech Language Pathologist to join our team!

Full-time

SLP $50-$65/HR

CF is $45 - $47/HR

Key Responsibilities:

• Resident Evaluation & Treatment Planning: Assess residents' speech and language needs and develop personalized treatment plans.
• Activity Selection: Design activities that help residents improve independence in their daily activities.
• Progress Monitoring: Observe and assess resident performance, adjusting treatments as necessary, and preparing detailed progress reports.
• Rehabilitation Support: Assist in the rehabilitation of residents affected by injury, illness, or other conditions.
• Maintain a Safe Environment: Ensure a clean, safe, and supportive environment for residents and staff.
• Confidentiality: Maintain resident confidence by ensuring the secure handling of confidential information.
• Documentation: Keep accurate and up-to-date records of resident care and progress, auditing department records as necessary.
• Team Collaboration: Work closely with healthcare teams to contribute to a cooperative and supportive team environment.

Requirements:

• Education: Graduate of an accredited Speech Therapy Program.
• Licensing: Current New York State Speech Language Pathologist License.
