Job Summary:

Anchor Health is seeking a compassionate and highly skilled RN Case Manager to join our team. As a vital member of our interdisciplinary team, you will provide expert care to patients with terminal illnesses, focusing on their physical, emotional, and spiritual well-being. Responsibilities:

• Provide comprehensive and ongoing assessments of patient and family needs

• Develop and implement individualized care plans in collaboration with the healthcare team

• Educate patients and families on self-care techniques, medication, and disease management

• Conduct regular re-evaluations and updates of patient and family needs

• Collaborate with community health-related persons to coordinate care

• Attend interdisciplinary group meetings and contribute to patient care planning

• Maintain accurate and detailed clinical records

Position Qualifications:

• Registered Nurse with a valid CA state license

• Hospice experience preferred; at least one year of recent hospice experience along with a strong foundation of acute care experience

• Valid driver's license, auto insurance, reliable transportation, and willingness to drive to patient locations

• Ideal candidates must have excellent interpersonal skills, motivated, and a passion for Hospice
