What You'll Do

As a Physical Therapist (PT), you'll evaluate residents and develop individualized treatment plans to improve mobility, strength, balance, safety, and overall functional independence.

Responsibilities include:

• Evaluating residents to identify mobility limitations, strength deficits, balance concerns, and functional impairments

• Developing and implementing individualized treatment plans focused on mobility, gait training, strength, and pain management

• Monitoring resident progress and adjusting treatment plans as needed

• Supervising and collaborating with Physical Therapist Assistants (PTAs)

• Recommending assistive devices and adaptive equipment to promote safety and independence

• Educating residents, families, and caregivers on treatment plans and mobility strategies

• Maintaining timely and accurate clinical documentation

• Participating in interdisciplinary team meetings and care planning

• Following applicable clinical standards, regulatory requirements, and facility policies

What We're Looking For

• Active Ohio Physical Therapist (PT) license

• License in good standing with applicable licensing boards and regulatory agencies

• Long-term care, skilled nursing, or geriatric rehabilitation experience preferred, but not required

• Net Health documentation experience is a plus

• Strong clinical, communication, and decision-making skills

• Ability to collaborate effectively with residents, families, and the interdisciplinary care team

• Passion for helping residents improve or maintain their independence and quality of life
