We are seeking a highly motivated and skilled Physical Therapist (PT) to work on a per diem basis in our state-of-the-art rehabilitation center. As a Physical Therapist (PT) at Pearl Nursing Center of Rochester LLC, you will have the opportunity to work with a diverse patient population, developing and implementing personalized rehabilitation programs to help them achieve their goals.

Key Responsibilities:

• Conduct initial evaluations and assessments of patients to identify physical therapy needs and develop individualized treatment plans

• Implement a variety of physical therapy techniques and modalities, including exercise, manual therapy, and education, to promote patient recovery and independence

• Provide high-quality patient care, prioritizing patient comfort, safety, and well-being

• Collaborate with other healthcare professionals, including occupational and speech therapists, to develop comprehensive treatment plans

• Monitor patient progress and adjust treatment plans as needed to ensure optimal outcomes

• Maintain accurate and detailed records of patient treatment and outcomes

• Participate in patient education and discharge planning, providing patients and families with the skills and knowledge necessary to manage their condition

Requirements:

• Current New York State Physical Therapy licensure

• Minimum of 0 years of experience in a rehabilitation setting

• Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and interdisciplinary team members

• Ability to lift 50 pounds and stand for long periods of time

• Valid driver's license and reliable transportation
