Admissions Director

We are actively hiring an Admissions Director to oversee admissions coordination, referral management, and resident transitions into a skilled nursing and rehabilitation setting. The Admissions Director plays a critical role in coordinating hospital referrals, reviewing insurance coverage, and ensuring prospective residents meet admission requirements. As an Admissions Director, you will work closely with hospitals, referral partners, residents, families, and facility leadership to support a smooth and efficient admissions process.

Key Responsibilities

• Manage incoming referrals and admissions processes as the Admissions Director

• Coordinate admissions from hospitals and other healthcare settings, ensuring timely and organized resident transitions

• Review referral documentation and coordinate pre-admission requirements

• Verify insurance coverage, benefits, and applicable payer requirements before admission

• Work with hospitals, discharge planners, case managers, residents, and families to coordinate admissions

• Utilize PointClickCare (PCC) to manage admissions information, documentation, and related workflows

• Maintain accurate admissions records and follow up on pending referrals

• Communicate admission decisions, requirements, and next steps to relevant parties

• Collaborate with facility leadership and clinical teams to determine admission readiness and support appropriate placements

• Build and maintain positive relationships with hospitals, referral sources, and community partners

• Support census growth and facility visibility through referral development and marketing activities as needed

Qualifications

• Prior healthcare admissions experience required

• Experience using PointClickCare (PCC) required

• Thorough knowledge of insurance coverage, payer requirements, and the hospital-to-skilled-nursing admission process required

• Skilled nursing, rehabilitation, or long-term care experience preferred

• Marketing, referral development, or business development experience preferred but not required

• Strong communication, organizational, and problem-solving skills

• Ability to manage multiple referrals and priorities in a fast-paced healthcare environment

• Ability to build positive working relationships with hospitals, referral partners, residents, families, and facility teams

• Strong attention to detail and commitment to maintaining accurate admissions documentation
