• Accurate Data Collection: Ensure the accuracy and completeness of MDS assessments, care plans, and updates by gathering and verifying information from residents, family members, and healthcare professionals.

• Collaboration with Interdisciplinary Team: Work closely with the interdisciplinary team to develop and implement individualized care plans that meet resident needs and promote optimal health outcomes.

• Communication and Documentation: Maintain accurate and detailed records of resident assessments, care plans, and updates, and communicate effectively with healthcare professionals, family members, and other stakeholders.

• Quality Improvement: Identify opportunities for quality improvement and contribute to the development and implementation of initiatives to enhance resident care and outcomes.
