Hospice RN Case Manager Hospice

REGISTERED NURSE - RN - CASE MANAGER - HOSPICE - Saginaw, MI and Surrounding areas:

Amara Hospice Care is seeking a Registered Nurse (RN) Case Manager to join our team of professionals serving Saginaw and the surrounding areas. The RN Case Manager will be responsible for assisting the team with case management/visits to ensure adequate coverage and call outs. Job duties may include:

• Completing an initial assessment of patient and family to determine hospice care needs
• Regularly re-evaluating the patients' nursing needs
• Using assessment data to determine care plan needs
• Collaborating with the entire healthcare team, patient, and family to direct the work of the team and provide continuous, well-coordinated care
• Counseling the patient and family in meeting end-of-life needs
• Initiating appropriate preventative nursing procedures
• Communicating with the physician regarding patient needs and reporting any changes in the patient's condition
• Supervising Hospice Aides and LPNs
• Participating in On-Call rotation when needed (extra pay)

The ideal candidate must have a current Michigan RN License in good standing, be familiar with and willing to travel the area, and have hospice experience. Additionally, reliable transportation and insurance are required. The schedule is Monday through Friday, 8:30am to 4:30pm, with care provided to hospice patients in their homes and facilities.
