Take your LPN experience beyond traditional bedside care with a unique mobile case management opportunity supporting adults through a Transitions of Care program. This role focuses on care coordination, patient education, follow-up, and reducing hospital readmissions while working alongside an RN-led multidisciplinary team.

Assignment Details

• Active NY LPN license

• Minimum of 1 year recent home health/case management or clinic experience
