About the Role:

As a Licensed Practical Nurse LPN, you will be responsible for delivering compassionate and individualized care to our patients. Your duties will include:

• Providing direct patient care, including administering medications, wound care, and other treatments

• Developing and implementing patient care plans in collaboration with the interdisciplinary team

• Monitoring and reporting patient progress to medical staff and families

• Participating in quality improvement initiatives to ensure optimal patient outcomes

• Demonstrating a strong commitment to patient-centered care and customer service

Responsibilities:

• Collaborate with interdisciplinary teams to develop and implement individualized patient care plans

• Administer medications and treatments as prescribed by physicians and other healthcare professionals

• Monitor and report patient progress to medical staff and families

• Maintain accurate and up-to-date patient records and documentation

Minimum Requirements:

• Current LPN licensure in the State of New Jersey

• High school diploma or equivalent required

• Minimum 0 years of experience in a healthcare setting

• Ability to lift, transfer, and move patients as needed
