Caregiver for Mid-Day and Evening Hours | Must fit client's schedule!

Continuum is seeking a compassionate, attentive, and dependable caregiver to provide one-on-one support for a female client in her home. The ideal candidate will be comfortable assisting an individual with mobility challenges, communication barriers, and safety needs while promoting dignity, independence, and quality of life. Success in this role requires patience, reliability, strong communication skills, and a commitment to helping this client remain safe and comfortable in her home.

Client's Caregiver Schedule:

• Monday: 2:30 PM – 8:30 PM

• Tuesday: 4:30 PM – 8:30 PM

• Wednesday: 4:30 PM – 9:00 PM

• Thursday: 1:00 PM – 9:00 PM

• Friday: 11:00 AM – 9:00 PM

• Saturday: 4:00 PM – 9:00 PM

Caregiver Responsibilities

• Provide companionship and meaningful engagement throughout the shift

• Assist with meal preparation and encouragement during mealtimes

• Provide medication reminders according to the care plan

• Perform light housekeeping

• Offer standby assistance and support with mobility and transfers to ensure client safety

• Utilize the client's available communication tools and techniques to support effective communication

• Monitor and report any changes in the client's condition, mobility, or overall well-being

• Encourage independence while providing appropriate hands-on assistance

Caregiver Qualifications

• Previous caregiving, home care, CNA, HHA, or direct care experience preferred

• Experience supporting individuals with mobility impairments, neurological conditions, or communication challenges is a plus

• Compassionate and skilled in adapting communication styles to meet client needs

• Able to remain attentive and safety-focused throughout the shift

• Comfortable providing physical assistance with standing, ambulation, and balance support

• Reliable transportation
