RiverSpring Licensed Home Care Services Agency is seeking a Registered Nurse (RN) Supervisor – Multi-Setting Clinical Role to join our dynamic team!

The RiverSpring Living campus is set on 32 acres overlooking the Hudson River, in Riverdale, New York.  RiverSpring Living provides a full range of care solutions including nursing home care, assisted living programs, senior housing, rehabilitation services and specialized services such as elder abuse prevention and memory care.

Position Summary

The Registered Nurse (RN) provides and coordinates professional nursing services across the Assisted Living Residence (ALR), Licensed Home Care Services Agency (LHCSA)/home care, and Skilled Nursing Facility (SNF) based on operational and resident/patient needs. The RN performs clinical assessments and reassessments, develops and evaluates plans of care, provides direct nursing care, supervises and supports nursing and paraprofessional staff, coordinates with providers and interdisciplinary teams, and promotes safe transitions across the continuum. The position supports clinical quality, regulatory compliance, accurate documentation, resident/patient safety, and person-centered care in all assigned settings.

Qualifications

• Graduate of an accredited school of professional nursing with a current, unrestricted New York State license and registration as a Registered Professional Nurse.

• Bachelor of Science in Nursing preferred.

• Minimum one year of nursing experience in long-term care, assisted living, home care, acute care, or post-acute care preferred.

• Prior supervisory or charge nurse experience preferred.

• Current CPR/BLS certification as required by organizational policy.

• Demonstrated knowledge of professional nursing practice, assessment, care planning, medication management, infection prevention, clinical documentation, and supervision of licensed and paraprofessional staff.

• Strong communication, organization, clinical judgment, computer, and documentation skills.

• Ability to travel between assigned settings and conduct home visits when required.
