Director of Social Services

Mohawk Meadows Rehabilitation & Nursing Center, a well-established skilled nursing facility in Lafayette, NJ, is seeking a compassionate and experienced Director of Social Services to lead our social services department. In this role, you’ll be the advocate ensuring every resident’s psychosocial needs are met — from admission through discharge — while guiding your team and partnering with families, clinical staff, and community resources.

What you’ll do:

• Oversee the social services department and ensure residents’ psychosocial, emotional, and discharge-planning needs are met

• Conduct psychosocial assessments and contribute to MDS and comprehensive care planning

• Coordinate discharge planning, referrals, and transitions of care with residents, families, and community agencies

• Advocate for resident rights and support families through placement, care decisions, and difficult transitions

• Ensure documentation and department practices comply with federal, state, and facility regulations

• Collaborate with the interdisciplinary team to support quality care and positive outcomes, working as a team player

What we’re looking for:

• Master’s degree in Social Work (MSW) from an accredited program — required

• Active NJ Licensed Master Social Worker (LMSW) licensure required

• Experience in long-term care, skilled nursing, or a healthcare setting, ideally in a leadership strongly preferred

• Strong knowledge of discharge planning, care plans, and LTC regulatory requirements strongly preferred

• Excellent communication, organization, and advocacy skills
