Key Responsibilities:

• Provide high-quality, patient-centered care to our residents and patients in a variety of settings, including rehabilitation and skilled nursing care

• Develop and implement individualized care plans that meet the unique needs and goals of each patient

• Collaborate with interdisciplinary teams, including physicians, therapists, and other healthcare professionals to ensure comprehensive care

• Administer medications, treatments, and therapies as prescribed by physicians

• Monitor patient progress and adjust care plans as needed

• Maintain accurate and detailed records of patient care and treatment

• Participate in staff education and training programs to stay up-to-date with best practices and industry developments
