Nurse Practitioner (PACE)

Make a Difference. Build Relationships. Transform Senior Care.

At Blue Ridge Care, we believe healthcare should be personal, compassionate, and centered around helping people live their best lives. As a mission-driven, non-profit organization, we are reimagining the way communities experience aging, serious illness, and end-of-life care through innovative programs and extraordinary care.

We're looking for a dedicated Nurse Practitioner to join our Program of All-Inclusive Care for the Elderly (PACE) team. If you're passionate about caring for older adults, enjoy building meaningful relationships with patients, and want to work alongside a collaborative interdisciplinary team, we'd love to meet you.

Why You'll Love This Role

As a Nurse Practitioner with PACE, you'll have the opportunity to provide comprehensive, patient-centered care to a vulnerable and deserving population. Rather than rushed visits and fragmented healthcare, you'll enjoy a more holistic approach that allows you to truly know your participants and impact their quality of life.

You'll work closely with physicians, nurses, therapists, social workers, and other healthcare professionals to coordinate care that helps seniors remain independent and thrive in their communities.

What You'll Do

As a key clinical member of the PACE team, you will:

✅ Conduct comprehensive assessments and wellness evaluations

✅ Provide primary care management for participants with chronic and acute health conditions

✅ Develop and update individualized care plans in collaboration with the Interdisciplinary Team (IDT)

✅ Collaborate with specialists, hospitals, caregivers, and community resources to ensure coordinated care

✅ Educate participants and families on health conditions, treatments, and preventive care

✅ Participate in team meetings and contribute clinical expertise to participant care decisions

✅ Maintain accurate medical records and documentation

✅ Support quality improvement initiatives that enhance participant outcomes

✅ Serve as a trusted clinical resource for staff and caregivers

What We're Looking For

Required Qualifications

  • Graduate of an accredited nursing program

  • Current Registered Nurse (RN) licensure

  • Current Nurse Practitioner licensure

  • National Nurse Practitioner certification

  • Minimum two years of experience working with elderly or medically complex populations

  • Current CPR certification

  • Valid driver's license, reliable transportation, and proof of insurance

Preferred Qualifications

  • Geriatric Nurse Practitioner (GNP) certification with prescriptive authority

  • Two or more years of Nurse Practitioner experience in geriatrics

  • Experience in PACE, long-term care, home health, hospice, community-based care, or other interdisciplinary healthcare environments

Success in This Role

You'll thrive if you:

  • Have a passion for caring for older adults

  • Value teamwork and collaboration

  • Possess strong clinical assessment skills

  • Are compassionate, patient-focused, and relationship-driven

  • Communicate effectively with patients, families, and care teams

  • Enjoy solving complex healthcare challenges and improving outcomes

What We Offer

We care for our team members the same way we care for our participants.

Benefits Include:

  • Medical, Dental & Vision Insurance

  • Retirement Plan with Company Match

  • Generous Paid Time Off

  • Paid Volunteer Time

  • Employee Thrift Shop Discount
