GEM Transitional Care Center in Pasadena, California, is seeking a motivated, experienced, and dedicated Director of Rehabilitation (DOR) to lead our in-house rehabilitation department.

We strongly prefer a licensed Physical Therapist (PT), but qualified Occupational Therapists (OTs) with strong skilled nursing experience are also encouraged to apply.

We are looking for a hands-on leader who is passionate about delivering exceptional patient care, supporting their therapy team, and building a successful rehabilitation program.

Key Responsibilities

• Oversee the daily clinical and operational functions of the rehabilitation department, including PT, OT, and ST services.

• Lead, mentor, and support therapy staff to promote teamwork, accountability, and clinical excellence.

• Manage staffing, scheduling, productivity, and departmental performance.

• Collaborate closely with nursing, administration, MDS, and interdisciplinary team members to ensure coordinated patient care.

• Monitor Medicare Part A and Part B therapy utilization and identify appropriate opportunities to meet residents' rehabilitation needs.

• Ensure timely evaluations, appropriate treatment planning, accurate documentation, and compliance with Medicare and regulatory guidelines.

• Monitor department performance, including labor expenses, productivity, and cost per treatment minute.

• Participate in interdisciplinary meetings, discharge planning, and quality improvement initiatives.

• Promote a positive, supportive work environment focused on patient outcomes and staff development.

Qualifications

• Current California Physical Therapist (PT) license preferred.

• Licensed Occupational Therapists (OTRs) with relevant experience will also be considered.

• Experience in a Skilled Nursing Facility (SNF) or post-acute rehabilitation setting strongly preferred.

• Previous Director of Rehabilitation, Assistant Director of Rehabilitation, or supervisory experience preferred.

• Strong understanding of Medicare Part A, Medicare Part B, and rehabilitation documentation requirements.

• Excellent leadership, communication, organizational, and problem-solving skills.

• Ability to balance clinical responsibilities with operational and financial performance.
