Quality Center for Rehabilitation and Healing is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide quality care to residents. The LPN will work with the nursing team to support residents’ health, comfort, and daily care needs in accordance with care plans and applicable requirements.

Responsibilities

• Provide nursing care to residents within the scope of the LPN license.

• Monitor residents’ health and report changes in condition to the appropriate nursing staff.

• Administer medications and treatments as assigned and in accordance with care plans, facility policies, and applicable requirements.

• Document care provided and maintain accurate resident records.

• Communicate with residents, families, and members of the care team.

• Support residents with daily care needs as appropriate.

• Follow infection prevention, safety, and confidentiality practices.

Qualifications

• Current LPN license in the state where the facility is located.

• Strong communication and interpersonal skills.

• Compassionate, respectful approach to resident care.

• Ability to work collaboratively with the nursing and care teams.

• Long-term care or rehabilitation experience preferred.
