Job Summary:

We are seeking a compassionate and skilled Home Health LPN to join our team at Orion Home Health in Humboldt Park, IL and the surrounding areas. As a Home Health LPN, you will play a vital role in providing high-quality, patient-centered care to individuals in the comfort of their own homes. Responsibilities:

• Provide skilled nursing care to patients in their homes, following established care plans and protocols

• Conduct patient assessments, develop care plans, and implement interventions to improve patient outcomes

• Work collaboratively with physicians, therapists, and other healthcare professionals to ensure comprehensive care

• Educate patients and their families on self-care techniques, disease management, and prevention strategies

• Document patient data accurately and timely, maintaining confidentiality and adhering to HIPAA standards

• Participate in case conferences, team meetings, and other activities to promote effective communication and coordination of care

Requirements:

• Current LPN license in the state of Illinois

• Minimum 1 years of experience in home health or a related field (new graduates welcomed to apply!)

• Ability to work independently and as part of a team

• Strong communication and interpersonal skills

• Reliable transportation and willingness to travel to patient homes throughout the Chicago area
