Responsibilities

• Provide personalized care and assistance to patients with daily living activities, such as bathing, dressing, grooming, and toileting

• Assist patients with medication management, including administration and documentation

• Perform vital signs, weight, and height measurements, and maintain accurate records

• Ensure patient safety and well-being by monitoring and reporting changes in condition to the nurse or physician

• Maintain a clean and organized living environment, adhering to infection control practices

• Collaborate with the healthcare team to develop and implement individualized care plans

• Communicate effectively with patients, families, and healthcare providers to ensure seamless continuity of care
