About the Role:

As a Home Health Registered Nurse with Orion Home Health, you will provide high-quality, patient-centered care to individuals in their homes. You will work collaboratively with our interdisciplinary team to develop and implement personalized care plans, educate patients and their caregivers on disease management and treatment options, and monitor progress to achieve optimal health outcomes.

Responsibilities:

• Conduct initial patient assessments and develop individualized care plans

• Provide routine visits to patients in their homes, including wound care, medication management, and education on disease prevention and management

• Collaborate with other healthcare professionals, including physicians, physical therapists, and occupational therapists, to ensure comprehensive care

• Monitor patient progress, identify potential issues, and adjust care plans as needed

• Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination

• Maintain accurate and timely records of patient visits and outcomes

Requirements:

• Current RN license in the state of Illinois

• Minimum of 1 years of experience as a Home Health Registered Nurse (new graduates welcome to apply!)

• Ability to work independently and as part of a team

• Excellent communication and interpersonal skills

• Strong assessment and problem-solving skills
