About the Role:

As a Home Health LPN, you will be responsible for delivering direct patient care, performing routine vital signs, and administering medications. You will work closely with our interdisciplinary team to develop and implement personalized care plans, promoting patient independence and well-being. This is a per-diem position, offering flexibility and autonomy in your schedule.

Responsibilities:

• Provide direct patient care, including vital sign assessments, wound care, and medication administration
• Assist patients with activities of daily living (ADLs), such as bathing, dressing, and grooming
• Develop and implement personalized care plans, working closely with the interdisciplinary team
• Communicate effectively with patients, families, and caregivers to ensure comprehensive care
• Perform routine checks and updates on patient charts and records
• Participate in team meetings and contribute to ongoing quality improvement initiatives

Requirements:

• Licensed Practical Nurse (LPN) with current licensure in the state of Illinois
• 1 years of experience in Home Health or related field (new graduates welcome to apply!)
• Ability to work in a fast-paced environment with a high level of autonomy
• Strong communication and interpersonal skills
• Ability to lift up to 25 pounds and move freely around patients' homes
