About the Role:

As a Home Health Registered Nurse, you will:

• Provide skilled nursing care to patients with various medical conditions, including wound care, medication management, and symptom control

• Collaborate with our interdisciplinary team of healthcare professionals to develop and implement personalized care plans

• Conduct initial and ongoing assessments to evaluate patient needs and progress

• Educate patients and their families on disease management, self-care, and healthy living habits

• Document patient information accurately and thoroughly in our electronic medical records system

Responsibilities:

• Travel to patient homes in Oak Forest, IL and surrounding areas

• Perform comprehensive assessments of patients' physical, emotional, and social needs

• Develop and implement individualized care plans to achieve optimal patient outcomes

• Collaborate with patients' primary care physicians and other healthcare professionals to ensure continuity of care

• Provide emotional support and counseling to patients and their families

• Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination

Requirements:

• Minimum Years of Experience: 2 years of experience as a Registered Nurse (RN)
