Orion Home Health is seeking a compassionate and experienced Field Registered Nurse (RN) to join our team in Chicago, IL. Responsibilities:

• Provide high-quality, comprehensive nursing care to patients in their homes, focusing on promoting health, preventing complications, and improving patient outcomes

• Develop and implement individualized care plans in collaboration with patients, families, and healthcare providers

• Conduct initial and ongoing assessments, documenting findings and communicating results to patients, families, and healthcare providers

• Administer medications, treatments, and therapies as prescribed, while monitoring patient response and reporting any changes or concerns to the healthcare team

• Educate patients and families on disease management, medication use, and self-care techniques

• Collaborate with interdisciplinary team members to ensure seamless care transitions and optimal patient outcomes

Requirements:

• Current RN licensure in the state of Illinois

• 2 years of experience as a Registered Nurse (minimum)
