Homeside Rehab is seeking licensed Occupational Therapists (OTs) and Occupational Therapist Assistants (OTAs) to provide home-based therapy services to patients in Brooklyn, NY, specifically within the 11219 and 11204 ZIP codes.

This is an excellent opportunity for therapists looking to add flexible, local home care cases to their existing schedules while working with a supportive clinical and administrative team.

Responsibilities

• Provide skilled physical therapy services in the patient's home

• Evaluate patient needs and develop appropriate treatment plans, as applicable to the OT role

• Provide treatment according to the established plan of care

• OTAs will provide treatment under the supervision of a licensed Occupational Therapist

• Monitor patient progress and communicate changes to the clinical team

• Educate patients and caregivers regarding treatment plans and home exercise programs

• Complete accurate and timely clinical documentation

• Maintain professional communication with patients, caregivers, and the Homeside Rehab team

Qualifications

• Active New York State Occupational Therapist (OT) or Occupational Therapist Assistant (OTA) license

• Minimum 1 year of experience preferred

• Home care experience preferred, but not required

• Reliable transportation

• Strong communication and documentation skills

• Ability to work independently in a home care setting

• Dependable and able to maintain a consistent patient visit schedule
