About the Role:

As an LPN with Orion Home Health, you will work closely with our interdisciplinary care team to provide high-quality, patient-centered care in the comfort of our patients' own homes. Your responsibilities will include:

• Administering medications and treatments as prescribed by physicians
• Assessing and monitoring patient conditions, reporting changes to healthcare providers
• Educating patients and their families on self-care techniques and disease management
• Collaborating with healthcare professionals to develop and implement individualized care plans
• Maintaining accurate and detailed patient records
• Maintaining a safe and clean environment for patients and staff

Requirements:

• Current LPN license in IL

• Minimum 1 years of experience (new graduates encouraged to apply)

• Ability to work in a fast-paced, dynamic environment

• Excellent communication and organizational skills
