Responsibilities:

• Provide direct care to residents, including assistance with daily living activities, medication management, and personal care

• Develop and maintain positive relationships with residents, families, and colleagues

• Observe and report changes in residents' conditions, communicating concerns to supervisors and healthcare professionals

• Participate in care planning, ensuring individualized care plans are implemented and updated as needed

• Maintain accurate and detailed records of resident care and services

• Collaborate with other healthcare professionals to ensure seamless care transitions and effective communication

Requirements:

• High school diploma or equivalent required
